Abstrak

It has been known that there are 80% of all accidents was caused by human error. The fact shown that 60% of that error were caused by mismanagement and 40% caused by sub standard action. The big number of shipping accident were caused by mistake. These occurred because of the lack of knowledge, skill, instruction or motivation from the people itself. That factors can be considered as mis procedural. Then, these can be concluded that all problem is the mistake of management system which arrange the shipping operation. With the modem technology, people could be able to decrease the technical error. But, however, with those modem technology, the management system which arrange people to act and to motivate to fullfil the instruction, is become a big problem. People behavior who work in system or company, consider safety aspect and prevention the pollute sea while shipping operation, depend on the company policy, procedure and work instruction. Because of all that matter, we need a management system which make a good and tight cooperation in shipping operation between land management and sea management for exceeding shipping accident etc. Management system must BE supported by a skillful human resources and a good supporting implementation. Based on this, in 1993 the IMO has published a new regulation known by ISM Code (International Safety Management Code) to standardize safe management and operation of ship for pollution prevention. Later, it also has become the Chapter IX in SOLAS-1974
